Abstract: Cirsoid aneurysms, although rare, represent an intriguing vascular pathology characterized by tortuous, dilated blood vessels with a propensity for arteriovenous shunting. While these lesions can occur in various regions of the body, their manifestation in the face (cheek) is exceptionally uncommon. This article presents case of 44 yr. male patient, suffering from recurrent Cirsoid aneurysm, rarely reported in literature. The main feeder for the aneurysm being Superficial Temporal artery and drainage to Internal Jugular vein. The Aneurysm was operated. Article involves a comprehensive review of cirsoid aneurysms, with a focus on their etiology, clinical presentation, diagnostic modalities, and management strategies. Through the exploration of a clinical case and pertinent literature, the article sheds light on the challenges associated with diagnosing and treating cirsoid aneurysms of the cheek.

Unravelling the Enigmatic: A Case Report on Type 1 Infected first Branchial Cyst and Surgical Management

Abstract:

Type 1 first Branchial cysts are uncommon congenital anomalies that often pose diagnostic challenges due to their varied presentations and potential for misidentification. We present a case report of a patient diagnosed with a type 1 first Branchial cyst, discussing the clinical manifestation, diagnostic approach, and successful surgical management. A 52-year-old female presented with a painful, recurrent, fluctuant swelling in the left lateral neck region, which was initially misdiagnosed as an infected lymph node. Imaging studies revealed a cystic lesion possibly arising from Parotid gland. Surgical excision was performed, and Histopathological examination confirmed the diagnosis of a type 1 first Branchial cyst. This case emphasizes the importance of considering Branchial anomalies in the differential diagnosis of lateral neck masses and highlights the significance of accurate diagnosis and timely surgical intervention for optimal patient outcomes.

Abstract: Artificial Intelligence (AI) has emerged as a transformative force in healthcare, presenting innovative approaches to data management, patient engagement, diagnostic accuracy, and surgical assistance. This study explores the multifaceted applications of AI across various subjects integral to the MBBS curriculum, aiming to identify its impact and potential in each domain. A novel approach was adopted using a chatbot powered by AI technology (ChatGPT) to streamline the data collection process. Keywords such as “AI in Healthcare,” “Telemedicine,” “Diagnostic AI,” “Surgical AI”, “MBBS curriculum”, “AI with name of branch” were systematically employed to search ChatGPT and compile relevant literature and data. The chatbot facilitated efficient data extraction and analysis, contributing to the synthesis of comprehensive insights. Surprisingly, our review revealed a dearth of prior studies focusing on the integration of AI across these medical subjects, underscoring the novelty and urgency of this research. This review not only sheds light on the current landscape but also paves the way for future investigations and advancements in AI-driven medical education and practice.

Analyzing Sensorineural Hearing Loss Among Family Members of Railway Workers Living Near Railway Tracks:   A Case Study

Abstract: This article investigates the prevalence and potential causes of Sensorineural Hearing Loss (SNHL) among individuals living near Railway tracks. Sensorineural Hearing Loss is a significant Public Health concern globally, affecting millions of individuals. While the etiology of SNHL is multifaceted, environmental factors, including exposure to Noise pollution, have been implicated. Railway tracks are recognized sources of Noise pollution, characterized by high-intensity Sound emissions from passing Trains. Audiometric tests were conducted on a sample comprising Railway employees and their family members who sought medical care for ear complaints at the Hospital. The participants were divided into two groups based on their proximity to Railway tracks. The collected data were analyzed to determine the relationship between distance from Railway tracks and the prevalence of Hearing loss. Analysis of the data reveals a significant association between distance from Railway tracks and the severity of Hearing loss. Participants residing further from Railway tracks exhibited lower rates of Hearing loss compared to those living closer to the tracks. The findings suggest a potential link between proximity to Railway tracks and Sensorineural Hearing Loss among family members of Railway workers. This underscores the importance of implementing effective Noise control measures and providing adequate Hearing protection for both Railway workers and their families living in close proximity to Railway tracks.

Benign Mixed Salivary Gland Tumour Arising from Anterior Nasal Septum: Radiologic Dilemma and Case Study

Abstract: Pleomorphic adenomas are the Benign Mixed Salivary gland tumours, typically found in major salivary glands. Their occurrence in the Nasal septum is exceedingly rare. This case report details the clinical presentation, diagnostic process, and treatment of a Pleomorphic adenoma originating from the anterior part of nasal septum in a 63-year-old female patient. The patient presented with a Nasal mass and Epistaxis. The initial diagnosis required the use of both Computerised Tomography (CT) and Magnetic Resonance Imaging (MRI) to accurately identify the tumour’s characteristics and extent. Complete Surgical excision was the treatment employed.

Abstract: This article examines three cases of Wegener’s Granulomatosis, now referred to as Granulomatosis with Polyangiitis (GPA), with a particular emphasis on ENT (ear, nose, and throat) manifestations and clinical observations. GPA is a rare form of Vasculitis that frequently affects the Upper respiratory tract, making early ENT evaluation vital for accurate diagnosis and treatment. The cases presented here illustrate a range of ENT symptoms, such as Chronic sinusitis, Nasal obstruction, and Epistaxis, which played a key role in the initial diagnosis. The detailed clinical assessments and imaging findings shed light on the disease’s progression and the difficulties in distinguishing it from other ENT conditions. This article underscores the essential role of the ENT department in the early detection of systemic diseases, emphasizing the importance of maintaining a high index of suspicion for systemic conditions during ENT evaluations. Abstract: The Prevalence of Gynaecology patients in Government hospitals, particularly in Durgapur, highlights significant Healthcare challenges and the accessibility of medical services for women in this region. This article explores the factors influencing the high number of Gynaecology patients in Durgapur’s government hospitals, analyzing both statistical data and broader social and economic trends. The study, conducted over six months with 460 enrolled patients, revealed that Reproductive tract Infections were the most prevalent condition, followed by Pelvic inflammatory disease and Menstrual disorders. These findings underscore the impact of poor health conditions, with infections being the most common issue, reflecting the region’s challenging socio-economic circumstances. The study emphasizes the importance of sustained government support and investment in healthcare infrastructure to address the increasing demand for Gynaecological services in Durgapur.

Abstract: Neck swellings are a common clinical presentation encountered in medical practice, varying significantly in etiology, pathology, and prevalence based on demographic and geographic factors. This study aims to assess the clinicopathologic features and prevalence of neck swellings in patients presenting to a tertiary care hospital in a Tier 2 city over a 1-year period. A retrospective observational study was conducted, analyzing clinical records, histopathological reports, and diagnostic imaging of 550 patients with neck swellings. Data were categorized based on age, gender, duration of swelling, and pathological diagnosis. The findings highlighted a diverse range of etiologies, including reactive lymphadenopathy, thyroid disorders, congenital anomalies, and malignancies, with notable differences in prevalence based on age and gender. Thyroid swellings were found to be the most common cause among adults, while lymphadenopathy predominated in pediatric cases. The study underscores the importance of early and accurate diagnosis for effective management and better clinical outcomes. This study provides valuable insights into the spectrum of neck swellings in a Tier 2 city setting, emphasizing the need for tailored diagnostic and therapeutic strategies in resource-limited environments.

Assessing the Mental Health of Doctors in Private Hospitals Using the General Health Questionnaire: A Survey Study in a Tier-1 City

Abstract: Doctors working in private hospitals often face significant occupational stress due to demanding work environments, long working hours, and job insecurity. The high levels of stress can lead to mental health issues, including anxiety, depression, insomnia, and social dysfunction. This study aimed to assess the mental health status of doctors working in private hospitals in a Tier 1 city using the General Health Questionnaire (GHQ 28). The GHQ 28 is a well established tool that screens for psychological distress across four domains: somatic symptoms, anxiety and insomnia, social dysfunction, and severe depression. The study surveyed 54 doctors from various specialties and found that 68% of participants experienced psychological distress. Anxiety and insomnia were the most affected do mains, with 82% of doctors reporting high stress levels. Job insecurity, which affected 61% of the participants, was identified as a major contributor to stress. The findings underscore the urgent need for regular mental health screenings, the implementation of supportive workplace policies, and the introduction of stress management interventions to improve the well being and resilience of healthcare professionals in private hospitals.

Anesthesia Techniques and Safety Considerations for Drug-Induced Sleep Endoscopy in Obstructive Sleep Apnea Patients: A Comprehensive Study

Abstract: Introduction- Sleep endoscopy, also known as drug-induced sleep endoscopy (DISE), is a diagnostic tool for evaluating upper airway obstruction in patients with obstructive sleep apnea (OSA). Anesthesia is pivotal in achieving a controlled sedative state that mimics natural sleep, ensuring patient safety and effective visualization of the airway dynamics. This comprehensive study reviews anesthesia techniques and safety considerations for DISE, focusing on optimizing patient outcomes. Methods- The study was conducted over one year with 100 patients clinically diagnosed with OSA. Exclusions included patients with neurological conditions, severe cardiac issues, and active nasal infections. Pre-procedure assessments included pre-anesthesia evaluations, blood tests, and imaging. Anesthesia was administered using Propofol titration, employing the Schneider model to achieve a snoring-apnea cycle. Standardized protocols were followed for patient positioning, monitoring, and sedation depth. Results- Propofol, administered in titrated doses, facilitated a controlled sedative state in all cases, allowing effective visualization of airway collapse. Moderate sedation was achieved in most patients, with minimal adverse events. Complications such as airway obstruction and respiratory depression were rare and managed promptly with established protocols. Discussion- The study highlights the role of anesthetic agents such as Propofol in achieving optimal sedation for DISE, ensuring airway patency and accurate diagnostic outcomes. Key considerations for anesthesiologists include precise drug titration, comprehensive patient monitoring, and readiness to manage potential complications like hypoxemia or aspiration. Conclusion- Anesthesia for sleep endoscopy is critical for the safety and success of the procedure. Adhering to evidence-based sedation protocols, tailored to individual patient profiles, enhances the diagnostic utility of DISE while minimizing risks. This study underscores the importance of anesthetic expertise in managing OSA through sleep endoscopy.

Patient Experience and Outcomes in the First Year of Conservative Management for Prolapsed Intervertebral Disc (PIVD)

Abstract: Introduction: Prolapsed intervertebral disc (PIVD) is a common spinal disorder causing pain and functional limitations. Conservative management, including physical therapy, medications, and lifestyle modifications, is often the first line of treatment. This study explores the experiences and outcomes of patients undergoing conservative management during their first year of PIVD diagnosis. Methods: A retrospective and descriptive analysis was conducted on patients diagnosed with PIVD who opted for conservative management. Data on patient-reported outcomes, pain levels (using a Visual Analog Scale), functional status (assessed via Oswestry Disability Index), and overall satisfaction were collected through interviews and medical records over 12 months. Results: Among the 100 patients studied, 75% reported significant pain reduction by the end of the first year, while 68% experienced improved functional abilities. Early compliance with physical therapy and adherence to prescribed medications were strongly correlated with better outcomes. However, 12% required surgical intervention due to persistent or worsening symptoms. Challenges reported included difficulty maintaining therapy routines, delayed symptom relief, and the emotional burden of chronic pain. Conclusion: Conservative management for PIVD can effectively alleviate symptoms and improve function during the first year for most patients. Early intervention, patient education, and tailored treatment plans are critical to achieving optimal outcomes. Addressing patient concerns and barriers to compliance can further enhance the success of conservative approaches.

Abstract- Laparoscopic Dismembered Pyeloplasty is the gold standard for managing Pelvi-ureteric junction (PUJ) obstruction, offering minimal invasiveness and excellent functional outcomes. However, the presence of Aberrant crossing vessels poses technical challenges, requiring meticulous dissection and vascular preservation. This study presents a case of PUJ obstruction with incidentally detected Double crossing arteries managed via laparoscopic Dismembered Pyeloplasty. We provide expanded technical insights, including intraoperative strategies for vascular identification, Ureteral Repositioning, and anastomosis optimization to ensure long-term patency. Additionally, we review the existing literature on anatomical variations and their impact on surgical outcomes. This report aims to enhance the understanding of complex PUJ obstruction cases and refine laparoscopic techniques for improved patient care.

Abstract: Endoscopic ear surgeries have always intrigued Otologists. However, the non-availability of the non-dominant hand for a simultaneous suction in the narrow ear canal has sought frequent answers from the established endoscopic ear surgeons. Professor Giuseppe Panetti has designed special endoscopic ear surgery instruments, to provide suction and instrumentation together in one hand. This study has been done to compare technique and results of endoscopic ear surgery done with conventional microscopic ear surgery instruments and Panetti’s set of instruments.

Materials & methods: This prospective comparative study was carried out at Department of E.N.T. & Head-Neck Surgery, Medical College, Baroda in duration of May 2022 to January 2023. 32 patients with ear discharge, decreased hearing and central perforation who have been operated endoscopically via Endo-meatal approach using conventional ear surgery instruments VS Panetti’s instruments, equally divided in two arms. Postoperative follow up was done at 1week, 4 week & 6 weeks.

Results: Separate suction canula was not needed as Panetti’s instruments has suction channel in these instruments. These instruments have 360 degrees rotatable Luer lock connector, which make these instrument’s use comfortable.

Conclusion: In the study comparing Panetti and conventional ear surgery instruments, no significant difference in surgery duration, graft uptake and post-op hearing outcomes was found. Surgical preference, cost, and instrument availability should guide selection.
